What the hell is a question you ask when you want to know why something occurred or what is going on.
Example: You would say "what the hell" when your teenage daughter comes in an hour after curfew with a older man.
Her: What the hell? Honey, why is there a lipstick in your coat pocket!?
Him: Oh, I found it outside and was going to give it to you. Because you're special! Surprise!
Her: Yeah right..

a phrase/reaction to something that is strange/abnormal to the person/you, usually when you see some weird shit.
Matt:what the hell? Mike & Angela are kissing?
Daren:now that's some weird shit, what the hell.
A phrase commonly used in polls. If you see "What the hell is this?" as a poll option, it would usually have a lot of votes.
Also means "What is this?," but funnier.
